Applications require proof of income, family size, and date of birth. Please refer to the list below for acceptable proof of income documents. Copies should be attached to your application and dropped off, or mailed, to our office at the address below. If you need assistance gathering documentation, please let us know.

TYPES OF PROOF OF INCOME:
Employment Income: (Provide a copy of any of the following)
  • Current Tax Return
  • Current W-2
  • Pay Stubs (most current 3 months)
Supplemental Income (Provide a copy of all that apply)
  • TANF (Current award letter or signed statement from caseworker)
  • SSI award letter
  • Child Support (official documentation)
  • Unemployment compensation documentation
  • SNAP eligibility documentation

Upon the receipt and review of your application, an enrollment specialist will verify eligibility. Eligible children will be placed if space is available, or added to the waitlist.

If your child is placed, an MCCC staff member will contact you to schedule an appointment to complete your registration paperwork. Please bring the following to this appointment. If you do not have one of the required documents at that time, we can assist in obtaining the information.

  • Up-to-date immunization records
  • Up-to-date well-child exam results
  • Current dental exam results
  • Medical cards or insurance information
  • Name and address of family physician
  • Name and address of family dentist

If your child is placed on the waitlist, you will receive a waitlist letter via US mail. If a space becomes available for your child, you will receive a phone call to schedule an enrollment appointment.

PLEASE NOTE:

Spaces are limited.

Acceptance into the program is based on need, not “first come, first serviced” with Head Start priority given to 4 and 5 year olds (not eligible for Kindergarten). Families are encouraged to apply as soon as possible, beginning February 1st of each year for the following school year starting in September.

An MCCC Family Service staff member will be assigned to guide your family through the entire application process.

Only complete applications can be processed for possible consideration for an open space.
